SPORTS PHYSICALS FOR SCHOOL YEAR 2010-2011
The Union County Health Foundation will hold sports physicals at the Alcester Medical Clinic in conjunction with the Alcester-Hudson Booster Club:
Who-5th grade boys
6th-11th grade boys and girls
Where-Alcester Medical Clinic
When-Wednesday, May 26th from 2:00-7:00 p.m.
Cost-$10.00 per person
Pick up forms at the high school office. ALL students MUST have all paperwork handed in to the high school office by Friday, May 21st5 in order to have a physical on the 26th. The 10.00 must be given to the clinic on the day of the physical.
The $10.00 for each physical will be given to the Alcester-Hudson Booster Club to help with expenses for students to go to team camps, Sanford Power Clinic, weight room supplies, etc. |
